Corpus Christi Medical Center is currently looking for a Position - Unit at our campus.
Orthopedic Nurse Navigator (ONN) is a member of the orthopedic care team providing patient centered, evidence based and holistic care for all orthopedic patients. ONN practices within defined scope of practice and in a manner consistent with the mission and goals of the organization. Proven track record of patient satisfaction and customer focus. Ability to interact with all levels. Must be proficient in computers and databases, and comfortable with new technology. Have superior interpersonal skill and ability to multi-task in customer service environment.
Reports to the Orthopedic Coordinator.
Specific Job Duties:
Assist patients in their understanding of their orthopedic surgery, what to expect from the surgery and how to prepare including the psychological aspects. Serves as an essential link between patients and other care providers, including PATs, necessary medical clearances, insurance authorization, patient education classes and other support services.
Primary liaison between key customers including but not limited to (patients, physicians, office staff, diagnostic staff, nurses, social service, case managers).
Follows patients throughout the care continuum, including pre-operative appointments, inpatient admissions, home care and outpatient physical therapy.
Collaborates with nursing staff and members of the clinical team ensuring plan of care for each patient includes appropriate interventions, patient education, discharge instructions, medication reconciliation, prescriptions and other appropriate information is provided to the patient before discharge.
Reduces the risk of canceled surgical cases by ensuring the timing of all necessary clearances and PATs . Helps patients understand their insurance coverage as well as the necessary durable medical equipment required for recovery.
Develops concise patient itinerary for use by the patient and the care team, and documents all communication with the patients and applicable care givers.
Encourages evidence based practice and assists in the development and implementation of order sets, clinical practice Guidelines (CPG's) algorithms in collaboration with orthopedic surgeons, orthopedic team members and order set committee.
Meets daily with orthopedic team members to ensure the plan of care for each patient is implemented and current and addresses patient care needs.
Provides orthopedic education to nursing staff and serves as a patient care resource.
Educates patients and significant others during hospitalization. Documents education in Interdisciplinary Education Record. Conducts preoperative orthopedic surgery class for total joint and spine surgery patients.
Participates, promotes and assists in the orthopedic program quality improvement initiatives.
Participates in hospital committees as required.
Participated in data gathering and interpretation with other member of the orthopedic team.
Registered Nurses with BSN required, MSN preferred with three to five years of experience. Orthopedic experience a plus.
