Full-time Description Job Title: Licensed Vocational/ Licensed
Practical Nurse Classification: Full Time/ Exempt Reports to:
Clinic Director Salary: $25-28 per hour based on experience
Summary: Provides support for medical providers and administration
to assure the smooth and productive operation of the health center.
Responsibilities: LVN/LPN Identify, coordinate and/or provide
appropriate levels of care under the direct supervision of an RN,
APRN, and MD Perform clinical operations and medical management
activities across the continuum of care (assessing, planning,
implementing, coordinating, monitoring, and evaluating). This
includes some level of case management and coordination of care.
Provide health education, coaching and treatment decision support
for patients. Verifies patient information by interviewing patient;
reviewing and/or recording medical history; taking vital signs;
confirming purpose of visit or treatment ahead of patients
scheduled appointment. Assists the physician by scribing notes,
referrals, follow ups, etc. Completes records by recording patient
examination, treatment, and test results. Triages Patient phone
calls for appropriate and timely follow-up. Assists with completing
of various forms, responding to insurance and other third-party
inquiries. Maintains safe, secure and health work environment by
following, and enforcing stands and procedures, complying with
legal regulations. Maintains patient confidence and protects
operations by keeping patient care information confidential.
Additional Expectations: LVN/LPN Learn and utilize EMR. Completes
task delegated by Providers and documents appropriately.
Requirements Qualifications/Experience: LVN/LPN Candidate must be a
Licensed Vocational or Practical Nurse. Spanish Speaking Preferred
experience in working in a FQHC or similar health center.
Experience working with LGBTQ Population Previous experience in the
treatment and management of HIV/AIDS. Time/Physical Requirements:
LVN/LPN Monday-Friday, 8:00am-5:00pm/40 hours per week., possible
10-hour shifts. Some evening and weekend activities may require
flexing schedule Tasks occur in both medical office and clinic
setting Able to lift 25lbs and perform normal duties associated in
a clinic and office setting.
