Based in San Antonio, TX, Team Care expanded its services to include Medicare-certified rehabilitation for retirement communities in 2001. The company operates a well-established pediatric outpatient facility and multiple senior clinics in nursing homes and assisted living facilities across San Antonio. With a dedicated team of 30 therapists, Team Care Rehab is committed to comprehensive and compassionate care. Role Description This is a full-time, on-site role for a Physical Therapist based in San Antonio, TX. The Physical Therapist will assess, plan, and implement individualized treatment programs to enhance patients' physical capabilities and quality of life. Responsibilities include providing therapy sessions for both pediatric and senior patients, monitoring progress, and collaborating with a multidisciplinary team to ensure optimal patient outcomes. Qualifications Strong expertise in Physical Therapy and Rehabilitation Experience with SNFs/ALs/LTCs, and the ability to manage ones time productively. Excellent communication and interpersonal skills Licensure as a Physical Therapist in the state of Texas Doctorate degree in Physical Therapy Prior experience working with senior populations is a plus Commitment to ongoing professional development Commitment to ones patients.
