Job Description:

Seeking experienced LVN to provide one-on-one care to patients in their homes, assisting with daily living activities and administering medications as prescribed.

**Responsibilities:**

• 
  
• Implement physicians' prescribed plans of care
  
• Conduct patient assessments and coordinate care
  
• Administer prescribed medications, treatments, and therapies
  
• Document care provided in a compliant and timely manner
  
• Teach and train family members on health promotion and disease management
  
• Accept and write verbal orders from physicians as required
  
• Follow up with physicians, clients, and family members on client problems and concerns
  
  **Qualifications:**
  
  • 
    
  • Active, unencumbered LVN license in the state of practice
    
  • Current CPR certification from American Red Cross or American Heart Association
    
  • TB skin test (current within last 12 months)
    
  • At least six months of prior hands-on nursing experience
    
  • Reliable transportation
