General Description
- The Medication Safety Pharmacist is responsible for safe
medication use within the health system. The Pharmacist is
primarily responsible for developing and implementing initiatives
directed at improving the safe use of medications for patients,
such as leading process improvement efforts designed to reduce
medication errors and/or improve patient safety at both the system
and departmental levels. Other activities include policy and
procedure development, providing education on medication safety,
providing education to staff on medication safety events, reviewing
and analyzing reported safety event, evaluating medication safety
systems and processes, and coordinating and monitoring intervention
strategies. The Pharmacist will also manage data reporting and
information related to medication safety and lead the medication
safety committee. In addition, the medication safety pharmacist
will assist the hospital with medication history review. The
Pharmacist collaborates with multidisciplinary teams to assure
appropriate documentation and submission required for designated
state funded programs. The Pharmacist will assist the Clinical
Manager with the ongoing management of policies, procedures,
practice expectations, and develop quality and performance
initiatives associated safe medication usage and investigational
medications.
Responsibilities
- Assists in coordinating departmental quality assurance
activities including drug utilization evaluation, medication error
review, adverse drug reaction and pharmacy intervention
reporting.
- Leads and maintains accountability for organizational efforts
to assess and optimize medication use system processes throughout
the continuum of care, in consultation with medical, nursing, and
pharmacy leadership.
- Leads medication safety error reduction initiatives.
- Serves as an authoritative resource on medication safety for
the organization by maintaining knowledge of trends and
developments in the patient safety field through continuous
professional development.
- Reviews and analyzes medication error reports and participates
in medication event huddles. Evaluates system and process change
recommendations at the department and microsystem level to
determine where medication safety risks and opportunities for
improvement exist
- Develops a plan for implementation of recommended
interventions.
- Serves on departmental and system level committees and
collaboratives pertaining to medication safety including leading
the Medication Safety Committee, and others as assigned.
- Serves as a member of the Pharmacy and Therapeutics Committee
(P&T). Committee responsibilities include analyzing and
reporting of adverse drug events and medication safety data to the
P & T Committee, and participation in medication related policy
development and implementation
- Completes necessary follow up actions including reporting to
outside agencies such as the FDA/MedWatch, VAERS and ISMP
- Leads and coordinates quality improvement and research to
identify and integrate best practices into medication use system
processes
- Conducts an appropriate and respectful interview of the
patient, or family member if necessary, to construct an accurate
list of home medications including the medication name, dosage
form, dose, route, frequency, strength/concentration, indication(s)
of use, and time/date the last dose was administered.
- Contacts internal and external healthcare providers and
pharmacy/pharmacies when needed to clarify or verify any
discrepancy in medication history or upon discharge.
- Identifies patient barriers to education about medications,
access to medications and/or compliance.
- Completes and enters audit data by defined timelines.
- Assures all audit data is thorough, complete, and
organized.
- Educates students, residents, fellows and others. Serves as a
preceptor for intermediate or advanced pharmacy students and PGY1
pharmacy residents
- Appropriately uses age-specific concepts and language when
communicating with employees, visitors, patients and others.
- Participates in the orientation and training of new pharmacy
employees as needed.
- Drafts new policies and procedures as needed.
- Effectively communicates with colleagues, and allied health
professionals.
- Liaises with external agencies/organizations, as required.
- Routinely checks all available means of communication including
e-mail, mail, and voice mail if applicable.
- Performs other related duties or special projects as
assigned.
Qualifications
- A Bachelor's degree or Doctorate in pharmacy from an accredited
college of pharmacy
- Texas pharmacist license
- Texas preceptor license
- Experience with medication safety and/or medication history is
preferred
