VA TEXAS VALLEY COASTAL BEND HEALTH CARE SYSTEM

The VATVCBHCS is comprised of the Corpus Christi Outpatient Clinic, Corpus Christi Specialty Clinic, Harlingen Outpatient Clinic, Health Care Center at Harlingen, Laredo Outpatient Clinic, and McAllen Outpatient Clinic. The VATVCBHCS is an ambulatory health care system providing health care in medicine, surgery, Mental Health, and rehabilitation medicine. The VATVCBHCS also operates a 100-patient Home Based Primary Care program and a 200-patient Care Coordination Home Tele-Health Program. Through the utilization of inpatient/emergency department contracts, approximately 2600 inpatient admission and 5800 ER visits are managed by VATVCBHCS.

Affiliated with the University of Texas Rio Grande Valley Medical School, the system has an active ambulatory care program and serves as a teaching and training site for medical residents. The VATVCBHCS is responsible for an enrollee population of over 30,000 Veterans and 350,000 outpatient visits annually.
The Women's Health/Primary Care Physician (PACT) functions as the physician leader within a multidisciplinary PACT team assigned to provide comprehensive, collaborative acute and continuity care to an assigned panel of Veterans as well as to patients outside of the assigned panel when Veteran care indicates a need for such.

As the Primary Care/PACT Provider will provide leadership in the application of the PACT model and clinical practice guidelines to organizational processes and/or systems in order to optimize patient care outcomes, achieve excellence in both internal and external customer service and shape collaborative, high-performing teams. Ongoing, continuous care of one or more assigned panel(s) of Veterans.Utilizing all available tools, such as registries, to enable effective and efficient identification and intervention of individual patients and cohorts.Ensuring appropriate evaluation and access is provided to patients assigned to the patient panel.Functioning at the full extent of the team member's relevant clinical privileges, credentials, scopes of practice, elements of practice, certification, functional statement, or other VHA or local facility approved documentation of competency.Participating in team performance improvement and sustainment activities to optimize team efficiency and care delivery to patients.Implementing primary care operations management processes, as appropriate.Managing communications and facilitating safe transitions of patients between the PACT's site of care and other health care settings, using informal and formal communication methods, as appropriate.Providing health education and health coaching on wellness, disease prevention, chronic care management, and self-management skills to patients and personal support persons, commensurate with the documented expertise or professional training of the PACT member.Engaging patients in using health care, encouraging patients to engage personal support persons, receiving input from patients and personal support persons regarding VA care.Using formal and informal communications that are respectful, effective, timely, and bidirectional with all team members (including the patient and personal support persons) to convey significant, clinically relevant information for the care of the patient.Providing health care commensurate to the PCP's licensure and clinical privileges or scope of practice.Ensuring the patient's care plan contains medical recommendations for clinically indicated care.Offering clinically indicated health care services to patients assigned to the PACT and providing or arranging for care to which patient's consent.Providing leadership to the team including shared delegation of appropriate care and care processes to appropriate team members.Reviewing available clinical and performance data with the team, focusing on continuous improvement of critical team processes.Ensuring the patient has same-day access for face-to-face and telephone care visits during regular clinic hours.Collaborating with PACT staff to develop personal health plans that incorporate care management and care coordination appropriate to the patient's needs.Communicating with facility leadership regarding the resources needed by the PACT for optimal function. As the Women's Health Clinical Champion (WHCC); you will be expected to meet the following performance standards including, but not limited to: Ensures that routine women's health care services such as but not limited to annual pap smears, conception counselling, contraception counselling, management of menopausal symptoms & breast cancer screening are completed for women Veterans in accordance with VHA directive 1330.01 appendix B.May serve as the Women's Health Medical Director within VATVCBHCS and perform duties in accordance with VHA directive 1330.01 appendix A.Collaborates with the Women Veteran Program Manager to promote & support Women's Health Care at VATVCBHCS.Third next available appointment within 14days - A minimum of 40% or more encounters as Video Connect or Telephone visitsEncounter completion with-in 24hrs of patient appointment visit - Access to and demonstrated use of the Primary Care Almanac to identify high risk patients for proactive interventionDaily attendance at PACT team let huddleAttends clinic and team meetings regularlyCompletion of Message Manager messages within 48hrs of message creationCompletion of Secure messaging response within 72hrs of message receiptMaintain unprocessed view alerts at no more than 200 on any given day and no unprocessed Critical view alerts at the end of each day.Ensure that all clinical reminders pertaining to HEDIS measures are completed at the time of the visit. (diabetes, prevention, ischemic heart disease, tobacco, depression/Alcohol/PTSD screening)Consistently demonstrates high quality collegial interactions with all staff across VATVCBHCS.Documentation of completion of mini residency and 20 hours of Women's Health CME within the last 3 years with 505% of panel population being female within the last five years or expectation of completion one employed.Duties assigned by Chief Medical Office Work Schedule: Monday - Friday (8:00 a.m. - 4:30 p.m.)
