Responsibilities We have an excellent opportunity for a Community Impact Director based across the Coastal Bend area . This is a part-time position working approximately 20 hours a week. Responsibilities The CI Director will drive the execution of health impact goals within the Coastal Bend area by focusing in the areas of hypertension, cholesterol, diabetes, nutrition, obesity, and physical activity. The market with have a focus on diverse and underserved communities. Drive market health assessments, organize andor lead community collaborations, build and implement plans for policy, system or environment change driving toward opportunities for health impact. Build and implement a cross-functional hypertension, diabetes, and cholesterol control strategy with measurable outcomes (high blood pressure management, cholesterol, diabetes management, and healthy behavior initiatives) in the market. Collaborate with development staff partners and volunteers to identify, cultivate and secure program funding, including sponsorship (cash and in-kind) for relevant programs, campaigns, and population health impact opportunities. Work with internal staff partners to ensure population health strategies are integrated into development, communications and marketing, advocacy, and healthcare quality improvement activities. Recruit, train and manage volunteers and strategic community alliances to achieve priority community and collective impact goals. Work with healthcare providers to ensure best practices for treatment of patients through initiatives and programs including but not limited to: Target: BP and Check. Change. Control. CHOLESTEROL & trade. Consult with clinical staff for data entry, data reporting, compliance issues, and strategies for improved patient outcomes. Other duties as assigned by supervisor. Qualifications Qualifications Bachelor's degree from an accredited university. 2+ years'experience working with communitypublic health issues andor managing communitypublic health promotion or related educational programs. Experience in population health a plus. Knowledge of the social determinants of health as it relates to behavior modification through self-care activities and policysystem-wide changes. Experience working with multicultural and underserved communities preferred. Experience in community led programs, projects, or events. Understanding of the benefits associated with use of treatment algorithms and their impact on clinical inertia. Understanding of continuous quality improvement methodology. Experience leading a QI initiative in a clinical setting a plus. Proven ability to recruit, mobilize and manage volunteers, including C-suite level executives. Ability and willingness to occasionally travel outside the market and to work evenings and weekends as needed. Ability to travel within the Coastal Bend area and transport items to and from meetings and events. Additional duties as assigned by supervisor.
